Referral Form
Submitting a case to Live Oak Veterinary Specialists is easy. Our Referral Form allows you to share patient details, medical history, and diagnostic results directly with our referral coordination team.
Please note:
- This is a professional contact form, not a secure medical portal.
- Veterinary medicine is not governed by HIPAA, but we treat all records and communications with the utmost respect for client and patient confidentiality.
- Uploaded files and case details are reviewed by our medical team only for the purpose of patient care and scheduling.
Once the form is submitted, our team will review it and contact the client (or your clinic, if preferred) within 24–48 hours to arrange an appointment.
